Plan Extras

Our Disease Management program is a free program for members who live with one or more covered chronic illnesses. Our Disease Management Nurses (Health Coaches), work directly with your Primary Care Physician in a team approach to closely monitor, improve and educate on diseases such as:

  • Diabetes
  • Congestive Heart Failure
  • Ischemic Heart Disease
  • Chronic Obstructive Pulmonary Disease/Asthma

As an enrollee of the Disease Management program, you may receive the following services and items, all cost-free to you. Services may vary by area:

  • Glucometers for diabetics to test blood sugar
  • Digital Scale for those with Congestive Heart Failure
  • Diabetic Shoes—No-cost diabetic shoes and insoles from Sole Support
  • Telephonic education — A registered nurse is only a phone call away
  • Educational Materials — Free information mailed to you
  • Rides to and from your Disease Management appointments at no cost to you
  • And more!

You can enroll in the Disease Management program by calling 1-800-494-6192 (TTY: 711), Monday – Friday, 8:00 a.m. – 5:00 p.m., Central Time. You may also discuss it with your Primary Care Physician, or download, print and fill out the program referral form.
