Physicians Health Choice includes extras to your plan, without having to pay extra. Benefits for the services below vary by service area and Plan. Refer to your Evidence of Coverage or contact Customer Service to see which extras are on your plan.

New 2012 Preventive Services

The following preventive services are now covered under your plan for a $0 copay when you visit your primary care physician:

  • Screening and counseling to reduce alcohol misuse
  • Screening for depression in adults
  • Screening for Sexually Transmitted Infections (STIs) and high intensity behavioral counseling to prevent STIs
  • Intensive behavioral therapy to reduce cardiovascular disease risk
  • Screening and counseling for obesity

SilverSneakers

The award-winning SilverSneakers® Fitness Program1, an exercise and physical activity program designed exclusively for older adults, is available to members of Physicians Health Choice at no additional cost.

The SilverSneakers Fitness Program includes:

  • A basic membership at a participating location
  • Access to any of the participating locations throughout the U.S.
  • Customized, low-impact SilverSneakers classes specially designed for older adults who want to improve their strength, flexibility, balance and endurance
  • Senior Advisor℠ assistance from a dedicated staff member
  • Health education seminars and other special events

Through Careington/FSL Dental, we offer routine dental cleanings, exams, x-rays and fluoride treatments at no cost to you. To provide these services, we partner with Careington/FSL Dental Network. Designed by two dentists, this network has more than 65,000 general dentists and specialists nationwide.

Mental health services are covered by Physicians Health Choice through MH Net, a behavioral health provider with over 20 years of experience. Our members are offered the treatment that includes a combination of care coordination, preventive health programs, and disease management.

RxPrescription Solutions® by OptumRx™

Prescription Solutions® by OptumRx™ provides convenient service and savings on maintenance medications. Maintenance medications are typically those drugs you take on a regular basis for a chronic or long-term condition, such as high blood pressure, high cholesterol, diabetes, and others.

  • Licensed pharmacists are available by phone 24 hours a day, 7 days a week to answer your medication questions.
  • Medications should arrive within seven days after Prescription Solutions receives your completed order.
  • No shipping for standard delivery to U.S addresses, including U.S. territories.
  • Prescription Solutions by OptumRx dispenses all medications into clearly labeled containers and mails them in a discreet, tamper-evident package.
  • Prescription Solutions by OptumRx is a national mail service pharmacy founded and managed by pharmacists.

Get started

  • By phone. Call Prescription Solutions by OptumRx at 1-877-889-6358 (TTY 711), 24 hours a day, 7 days a week. Please have your medication name and physician’s telephone number ready when you call. They can get you set up with home delivery right over the phone.
  • Or by physician. Have your physician’s office call 1-800-791-7658 with your prescriptions, 8 a.m. - 8 p.m. CT, Monday - Friday, excluding certain holidays, TTY 711 or fax them to 1-800-491-7997 24 hours a day, 7 days a week.
  • Or by mail. Complete and mail a prescription order form (PDF) to Prescription Solutions by OptumRx.

Comfort Care Transportation

Comfort Care offers cost-free transportation to and from your medical appointments. Please contact Comfort Care with your request for a ride at least 48 hours before your scheduled appointment.
