Our Standards

Physicians Health Choice takes a unique approach to contracting with physicians. We believe our network should be made up of providers who practice medicine with preventive measures and personal attention.

For Primary Care Physicians, this includes:

• Visiting with new patients within 90 days to understand their health needs
• Visiting with established patients at least once annually
• Closely monitoring chronic conditions such as diabetes and heart disease
• Following up with patients soon after hospital discharge to review discharge instructions and prescription medications
